A fee schedule is included at the end
of this document. Fees may be changed at any time
without prior notice. My charges are based upon the
usual and customary charges and reflect the cost of
my office expenses. The fee schedule reflects what
is charged to both individuals and insurance carriers.
Many carriers have contracted adjusted fees with me
as a preferred provider. Your billing statement will
show the amount that is written off of the account
and the contracted fee by your carrier.

Therapeutic Family Mediation - payable at time
of service, legal consultations with attorneys,

related to mediation cases will be billed at a
rate of $175 per hour in 15 minute increments
and must be paid fully at the close of each month
or additional services will be suspended until
payment is made in full. |

Collection Fees are added at 35% plus all court
and legal costs for accounts not paid within 6 months.
Payment plans available based upon need. Credit cards
accepted.
You may pay your account by cash, check
or credit card. Cashiers checks may be required if
returned checks is a problem and for large sums of
money. Please do not be offended if you are asked
to present a bank check or money order for payment,
this is simply a good business practice. Returned
checks will incur a $40 processing fee and all bank
and applicable attorney’s fees. Accounts not
paid within 90 days of receipt and completion of
payment from third party resources are subject to
being turned over to a collection agency for collection.
If it is necessary to file legal charges to receive
payment, you are responsible for all court cost and
attorney fees necessary until the account is cleared
in full. If your account becomes more that 120 days
delinquent without any communication or attempt of
payment, your account will be turned over to
a collection agency to collect payment and a surcharge
of 35% of the total balance will be added to collect
the debt plus 21% interest on the total charge until
the account is paid in full.
